Researchers at the University of Colorado Anschutz reported evidence linking circulating exosomes to persistent nerve irritation and lingering pain after shingles, a condition known as post-herpetic neuralgia (PHN). The study found that exosomes collected from PHN patients triggered neuroinflammatory and remodeling changes in lab nerve cells resembling those seen during viral infection. The work proposes a “failure-to-resolve” model in which the body does not fully shut down the healing response after varicella zoster virus clearance, with exosome cargo continuing to deliver maladaptive signals. Investigators reported the findings in Annals of Neurology. By identifying circulating exosome-mediated mechanisms as mechanistic contributors, the study opens a path toward new biomarker strategies and targeted therapies aimed at disrupting persistent exosome signaling rather than focusing solely on initial viral injury.
