A phase II pilot trial reported that a single fecal microbiota transplantation combined with atezolizumab and bevacizumab was safe and produced tumor responses in about one third of liver cancer patients whose disease had resisted immunotherapy. The study positions intestinal microbiota manipulation as an intervention lever for improving checkpoint efficacy in hepatocellular settings. For pipeline developers, the trial adds to the growing evidence that immune resistance can be bi-directionally influenced by microbial ecosystems. The key next steps will be identifying response predictors and standardizing FMT or microbiome delivery strategies to enable reproducible patient selection.
