Karyopharm’s Xpovio (selinexor) lost another clinical expansion bid in endometrial cancer, with Phase 3 results failing to meet the progression-free survival primary endpoint. The company’s latest readout, XPORT-EC-042, triggered a sharp selloff as it also highlighted limited near-term cash runway. Karyopharm said it is exploring financings and strategic alternatives while continuing plans to file an sNDA in August. It is also pursuing an accelerated pathway in myelofibrosis—requesting priority review based on overall survival data in the Phase 3 Sentry trial with Xpovio plus Jakafi (ruxolitinib). The combination of a second Phase 3 setback and limited funding options raises the likelihood of broader restructuring decisions, even as management continues to rely on the myelofibrosis rationale to sustain the program portfolio.
