The LAVA-CRLM trial has delivered a first direct comparison of two curative-intent options for colorectal liver metastases, with radiation matching heat-based ablation. In the randomized study conducted at Copenhagen University Hospital, stereotactic body radiotherapy was pitted against microwave ablation, closing an evidence gap that previously left clinicians choosing between approaches without head-to-head data. The trial’s completion is especially relevant in metastatic colorectal cancer because local control strategies can determine whether patients remain candidates for long-term survival pathways. The comparison also provides a structured benchmark for sequencing and technology adoption in liver-directed care. Clinicians will now look closely at endpoint reporting from LAVA-CRLM to interpret tradeoffs in local failure, toxicity, and overall treatment selection.
