The FDA approved talazoparib (Talzenna) in combination with enzalutamide (Xtandi) for adults with metastatic androgen pathway modulation-naïve or -sensitive prostate cancer carrying homologous recombination repair (HRR) gene mutations. The decision was supported by TALAPRO-3, a randomized, double-blind, placebo-controlled Phase 3 trial enrolling 599 patients. The primary endpoint—investigator-assessed radiographic progression-free survival—showed a hazard ratio of 0.48 (P<.0001), with median rPFS not reached versus 45.8 months for the placebo arm. Overall survival data were still immature at the time of analysis, but the FDA’s approval expands the frontline therapeutic role for PARP inhibition across a defined HRR-mutant population.
