A systematic review presented at the Endocrine Society’s annual meeting in Chicago reports preliminary evidence that GLP-1 receptor–based obesity drugs may increase testosterone levels and improve sperm quality in men with obesity, though data remain limited. The review team, led by Pratibha Natesh of Warwick Medical School, identified only five randomized controlled trials that measured testosterone. In one study, 30 men with hypogonadism and obesity received either a GLP-1 drug or testosterone replacement therapy (TRT). At 16 weeks, testosterone rose in both groups. In another trial (25 men with type 2 diabetes and hypogonadism), testosterone increased in both arms over 24 weeks, with a greater rise in the TRT group but with sperm quality improvements observed in the GLP-1 arm. The authors emphasized that the evidence base is small and called for larger, more definitive trials.
