A head-to-head comparison found gallium-68 labeled FAPI-04 PET/CT detected liver metastases with higher sensitivity and accuracy than fluorine-18 FDG PET/CT across a wide range of malignancies. The study reports stronger performance for identifying metastatic spread to the liver, where accurate staging is often decisive for treatment selection. If validated across additional cohorts and indications, the data could shift imaging workflows toward FAP-targeting tracers for patients being evaluated for hepatic involvement. The results also provide a clear comparative benchmark against the long-standing FDG standard.
