The researchers propose a “failure-to-resolve” model in which the nervous system does not properly shut down the healing response after varicella zoster virus clearance, leaving exosome-mediated signaling to sustain maladaptive remodeling. The findings shift the mechanistic framing for chronic shingles pain away from damage alone and toward ongoing molecular communication—creating targets for prediction, prevention, and therapy development based on exosome cargo.
