CVS Caremark changed its formulary positioning for Eli Lilly’s obesity portfolio, placing Foundayo (oral tirzepatide/GLP-1–based therapy per Lilly branding) on formularies June 1 and adding back Zepbound as a preferred option starting Oct. 1. Novo Nordisk’s Wegovy pill and injection both remain preferred. Lilly said the update effectively gives both of its covered options access through all three major U.S. pharmacy benefit managers, while noting individual employer plans may still opt out of coverage. Leerink Partners analyst David Risinger said the decision reverses Novo’s prior formulary advantage on CVS and expands the addressable prescription pool for Zepbound and Foundayo. The move arrives after earlier PBM-linked market-share pressure in GLP-1 formularies, and it provides a near-term test of how quickly patients and prescribers can move when preferred status changes across national payor networks.
