Cretostimogene grenadenorepvec delivered durable activity in a phase III BOND-003 cohort C trial for high-risk, BCG-unresponsive non–muscle-invasive bladder cancer with carcinoma in situ, according to results published by Tyson and colleagues in The Lancet Oncology. In the international single-arm study, 83 of 110 evaluable patients (75%) achieved a centrally confirmed complete response at any time after a median follow-up of 25.8 months, far exceeding the prespecified historical benchmark of 20% (P<.0001). Responses included additional complete responses after re-induction, with 46% achieving complete response at 12 months. Treatment was administered intravesically once weekly for 6 weeks, with an option for a second induction course at 3 months for persistent disease, and maintenance following complete response. Safety signals were predominantly mild and transient in the safety population of 112 patients. The trial reinforces the use of oncolytic immunotherapy in earlier-stage, high-risk bladder cancer settings where options are limited after BCG failure.
